Hi cox,

1. Put the script - without the <script> tags - in the ChronoForms JavaScript box.

2. Put the form html - without the <form> tags - in the ChronoForms Form HTML box.

3. Change the form name in the script from 'calc' to the ChronoForms form name - probably 'ChronoContact_calc'

4. Complete the remaining fields as necessary.

Did you try to look at the error using Firebug the Firefox extension ? this will help you much!
