I found that if you have Gzip compression turned on and try to create a new form it directs you to index2.php and the page is blank.

I had to turn that feature off to get it to work.

Hmmmm, Anyway Joomla 1.5 is still has lots of issues I see, may be this is an issue with joomla itself, I will check and let you know if i found anything!!🙂
