It's almost impossible to help you without knowing excatly what is going wrong.
Please feel free to post in your own language if that helps and we'll try to understand.
It woudl also help if you can upload a backup of your form so that we can look at it in detail. If not, then images of the General and File Uploads tabs would help.
Yes the Form tag attachment field is the one on the General tab labelled "Form tag attachment: something like onSubmit()"
