I want to do a simple form like this:

The background color is to separate the selection of user data. I added it to differentiate.

I used ChronoForms v4. (validate🙂)

Can anyone help?

Fieldsets are probably the best answer to this (with a bit of CSS). Unfortunately ChronoForms doesn't provide fieldset elements but you can dot them with Custom Elements - please see this post for a way to do it with Custom Elements.
