variable form

Hello,

I would like to create a form with fields that appear when certain choices.

For example, there is a radio button question: would you like your date of birth yes or no.
If they choose "yes" then a box will be where it can fill, if they select "no" should not appear and you can continue with the rest of the form

Is there anyone who can help me here with

Which version of ChronoForms are you using? You can find the version from Site Admin | Extensions | Install/Uninstall | Components in Joomla! 1.5 or Site Admin | Extensions | Extension Manager | Manage in Joomla! 1.6.
