Two Forms in the same page doesn't work

Hello, I've installed chronoform on my web site: http://www.golflife.it
In the home page there are two different forms. The problem is that when I compile a form (the bottom one), the other one will find an error. These form, for some reason, are in communication; so a person can't compile a form and send information without compiliing the other one.

The two forms need to use different input names and ids.

It used different ids and names, but it doesn't work with the latest version. Until previous version all was ok and i used two and three forms in the same page, but with this version (v.4) I've found some problem. Please Help Me, i need it for the work's appointments
