How to design a form with database field combo box?

Hi!!!
First of all... I'm a nob on web design and programing, but I need to do the web for a non-profit organization
I'll try to explain what I'm trying to do....

I have to make a form to monitor the status of hearing dogs, for that I need to create a form where enter the name of the dog, then use that field from database in another form (on a combo box) in which will be able to select the dog and with the rest of the form monitor the dog status (training, healh, etc...).

I'll need to make a report finally with the values that be on the form (all of them by date)

I have no problem on create the forms😀
I have no idea that how to make the conection betwen the form and database
I didn't work on the report, but i hope that will be able to do for myself.
