I'm trying to upload a file (.jpg), when I press the botton "send" everything look ok but nothing arrive on my FTP server and no attach file on my email.
Do you have any suggestion?

The video should give you want you want - I think the problem is that you have field1 in the files box and not the name of your file upload input.
