Chronoforms/CBRegistration Interference

As background information, the site uses CBRegistration for Member Login on a sidebar on every page. After installing Chronoforms 3.2 on this Joomla 1.5.0 website, I built a webinar sign-up form. When displaying the form page, the form shows up in two places. One is crammed into the 'Member Login' div on the sidebar, the other is displayed correctly in the body of the page. Of course the form is unusable.

Is there something I missed? I do not see any issues in the template, the CB form or the ChronoForms form. Example at the address below.
