No email sent and question about attaching files to email

Question 1: Can somebody please se what's wrong with my form? I guess it's something with the settings for files upload.

I don't receive any emails and I don't see my Thank you-page after clicking "Submit".

E-mail from the site works.
E-mail is the same as in the global settings.
Form method=File

I use Joomla 1.6.5 and Chronoforms V4 and I've made the form with the Eazy Form Wizard.

Question 2: Is it possible to attach the files uploaded to the email?

Yes, you can attach the files to the email. Add the input names in the "Attachments fields name" box in the Email configuration.
