error with submit & db

hello to all ..

I have an error and can not find the solution to the problem.

everything works fine .. I tried different ways and forms and manages to find the problem ..

the problem is up the next:
when you enable the option "save to db" throws me up the next mistake

obviously leave everything configured properly set the table with the wizard and too well but does not meet the function and throws me the error

I can not find the solution .. rather than disable the option .. and clearly not the idea that is what I need .. I can do?

The URL looks like a mess - are you using an OnSubmit URL in your form? Or a redirect?

solve the problem but I found the problem. . does the following

a form with their data (with user instructions) = bad form


another form with the same data but ran insturcciones user = ..

I think this could be taken as a "bug" .. Greetings! and thanks!
