2 forms are sent simultaneously

Hello Encountered the following problem:

The page contains two forms. They are configured to send mail to the mailbox. When filling out and sending one of them came to me two(!) letters - from the first and second. In first letter that I entered (that is OK), and the second blank fields in the form of {input_textbox_0}, etc.
How can I be? How to turn off simultaneous transfer of data between two forms?

p.s. when sending a form appears in both 'Thanks message'

problem solved. there was a conflict jquery and mootools. Thanks for ur job!
