Help, website not displaying after creating a simple form

Hi all, please I need an urgent help. Just downloaded chrono-form. Created my first simple form. When I click on frontend view it shows Form name can NOT be empty!. Even my home page is displaying the same message, the contents of my website does not show at all. I have other forms created with CK-forms all have been working prior to my installation of the chrono-form

Please direct me. I am new to joomla too.

Sorry but it's impossible to tell what is wrong from your description. Please post a link to the form so I can take a quick look.

Hmmm. Not something that I've ever seen before.

I'd go into the Admin and unpublish the form - or delete it if necessary and start over.

Hi, the form is working fine thanks. But the form is displaying at the buttom page, I want to bring it to the top. Any idea where to adjust the layout? Thanks
