Title Tag disappears when enabling Joomla 1.5 SEO options

I have a huge problem, I have chronoforms installed in the Latest Joomla! 1.5 which works perfectly, my problem is that whenever I enable the standard Joomla! SEF (SEO) in the global configuration file and go to the chronoforms form menu on my website the title tag that I have defined within the Menu item takes on the menu name and my own defined title tag is missing, however whenever the SEO is switched off everything is normal, the other issue is that the breadcrumbs also dont show the form menu item name, instead it shows Home when the SEO is turned on.

All my other menu items/components display perfectly. Any help advise will be appreciated.
