Form Preview Problem

Hello, I have a small problem with a form I made - the preview is a blank page

I also have integrate it in a article and it shows "There is no form with this name or may be the form is unpublished, Please check the form and the url and the form management. " but I'm sure the name is correct

Which version of ChronoForms are you using?

If it is CFv4 then you need to drag a Show HTML action to the OnLoad event to display the form fields.
