Module not working on the home page

I am using ChronoForms V4 on Joomla 1.6 . For some reason the form is working on every page but the default page. When the form is used on the home page I get a 404 - Component not found error page. This site can be viewed at

There appears to be a problem with the Joomla! URL handling on the home page that doesn't play nicely with ChronoForms URLs. Another poster has found a good diagnosis but we don't yet have a fix.
