Date picker not working...?

Hi guys.
I've added a V4 form on my site and everything works great, except the date picker doesn't seem to be working. Is there something I need to activate?

This works OK on the form without the template so there is a conflict with something else on the page :-(

There's an error "Uncaught TypeError: Cannot call method 'extend' of undefined" at line 451 of ja.ddmos.js - it looks as though the Drag.Base Class isn't loaded. From a quick Google this looks like a problem with JA Purity 2 and MooTools 1.2
