I am still struggling with my form on V3.2.0

I thought I had the upload file sorted out for jpg's only and happily have sent several test emails from my site, none have arrived though. I must have missed something, somewhere, but no idea what, or where. The tutorial doesn't look the same as the version I'm using and I built mine using the wizard. My Global settings are set to allow the upload - it is probably blatantly obvious, but not to me.

Does the form need to be validated first?
Is this correct for my upload files?
