When clicking submit, nothing happens, form gets reset. There is supposed to be a thank you page. It used to work and go toward a thank you page. But I must have tweaked something somewhere, and I did connect it to a table afterward. Any suggestions?

Yes. Thank you. The issue was that it somehow was not recognizing the "thank you" page's URL and was defaulting back to the form, or to the next form. If anyone else has a similar problem, test by putting in some simple URL into the thank you page, and see if it works.
