How can i do like this using chrono form

I would like to thank you guys for the best form extension. i would like to ask you guys b/c i have no idea about CSS and PHP programing so how can i make a form like this? to collecting information from users.

This is a pretty standard HTML table layout. You can create this with any HTML editor, then copy and paste the Form HTML (without the <form> and </form> tags) into the ChronoForms Form HTML box.

Any will do and there are many around. kompoZer is one possibility.

However this isn't a good point to start learning HTML, you might be better finding a knowledgeable friend to do that bit for you.
