Upload File not sending attachment & Internal Server Error

I've set up a form with an Upload File option.

The emails are being sent, but without the attachment.

Also, it generates the following message...

Internal Server Error
The server encountered an internal error or misconfiguration and was unable to complete your request.

Please contact the server administrator and inform them of the time the error occurred, and anything you might have done that may have caused the error.

How do I get the attachments to send and fix the Internal Server Error message?
