Problem with table database

Sorry if I upload a new post without looking for solutions in previous posts.
I do not speak English, so not easy for me to do this.

I made a form with chrono forms that you can see or try here:

I created an e-mail to the user and an e-mail informs me that its registration.
I created a table
I enabled all the fields in my form (but I left them all as "Vacher 255")
I connected the database to the form.
Then I published the form in a module and then in an article.

The problem is that after submitting the form, the user is in a blank page is no template.
Furthermore, the mail comes to me for inclusion, but the user do not get the confirmation email.
More, data are not stored in the database table.

If disconnecting the form from the table, everything works.
There are turorial I can follow the link to the form of tables?
If anyone has made to the tutorial in Italian?

Hi isma22,

You have a dash in the 'e-mail' input name. Dashes give ChronoForms and MySQL problems. Please rename the input using only [a-z], [A-Z], [0-9] or underscore '_'. No dashes, spaces or other special characters.

You also have to rename the e-mail column in the database table and then refresh the DB Connection (see below).

You need to refresh the DB Connection after any changes to database column names. In the Form Editor click the DB Connection tab and set the Connection to 'No'. Click the 'Apply icon in the toolbar to save the form, open the DB Connection tab, set the Connection back to 'Yes' and re-save the form. This will refresh the copy of the table information that ChronoForms uses.
