Form does not publish while settings are published

I am on a Zephyr template on Joomla 1.5.22 and I was successful in installing ChronoForms_V3.1_RC5.5
Created my form according to instruction, but it doesnt show me the form. Can anyone help me out? :?

I have published the form but it doesnt show.

Yes I got it, it's evening here and I was away from the computer.

It's fixed. The form is fine. The problem is with AceSEF which had created a duplicate SEF URL that went to an non-ChronoForms link. I've made the 'employment' link a custom, locked link and deleted the duplicate.

You may need to watch this - AceSEF is very nice but occasionally too clever. On one site I've had to turn off the auto-generation as I was getting too many duplicated and broken SEF URLs.
