Stumped... CSS issue with RocketTheme Joomla template

Not sure where to begin so I'll start by just showing examples.

This is on a church website, http://www.gracereformed.net/ .

Early in December I upgraded the Joomla template to one of the recent RocketTheme offerings. Everything was working as expected.

I logged in this morning upon hearing that the contact form was down. Sure enough, I found that the Submit buttons had somehow been disabled. I re-generated the contact form from the wizard, as well as a secondary form we're running for the next few weeks, which restored the submit button.

Now, however, the form is filling up the entire browser window, hiding the logo and other modules even though a quick glance at the source code shows they're being loaded.

The forms work now, but appear broken in that the site banners and logo don't show up. I need help from someone with more knowledge of ChronoForms than I have. Seems to be something funny with the CSS or form code generated within ChronoForms.
