Click Radio button then open new form

My problem is very critical..

problem is:

1) i use Chrono Contact version 3.0 STABLE
2) also use chronocontact plug in version 0.8/V3.0 Stable
3) My form is very long so in this form at last two radio button is display code of that radio button is...

                          <div class="clear"></div>
                        </div> </td>

4) when i click on 1st radio button then open a new form and click on 2nd button then open submitt button ...

5) submit button code is same for both of the button ...

in short when i move another form with javascript and this effect is work at that time when i click radio button...

please help me BOB my site dead line is after two day..please..

when i put the JavaScript code and then it's work

any other plugin or module is required????
if yes then name of that plugin or module or link..
or any version of chrono contact is solve my problem please help me...
