I have searched on these forums for my particular problem, but I have not seen a post that could resolve my issue. For some strange reason my checkboxes on my form are set to display: none as a style labeled in the input for each checkbox. Therefore the checkboxes do not show, but the label does? I have used Firebug on Firefox to determine that it is an element style and not anything in a CSS file. Of course if I use Firebug to edit the input I can take out the display:none and the box will appear next to the label. What is strange is if I view the form code, or even the view source code of the page it doesn't seem to have the display style actually listed on the input code. So I am not sure how I could take this off unless this is something I have to do in the admin? Would anyone have any suggestions?
Hi illfxdesigns,
You probably have a RocketTheme template. If you turn off Input Formatting in the template Advanced section then all will be well again.

That seems to work! Thanks
Bob, always happy to come back to the forum and search for answers. I just came across the same problem (I'm using RocketTheme too) and solved it in an instant with your post. THANKS!
