It WAS working.
remember this is A Joomla 1.0.15 site.
i verified the form is there and public, and clicked the link in the backend in the forms list and no matter what link i use, direct, or using the backend link
i get the message:
"The page you are trying to access does not exist."
"Please select a page from the main menu. "
I have recreated the link in the main menu both from the link type menu item as well as using component type of menu items. same result.
then i created a copy of the form and this is what i got.
name: Contact-Belaty_29112010_180649u
link: index.php?option=com_chronocontact&chronoformname=Contact-Belaty_29112010_180649u
what kind of link is THAT? can a change in OS or PHP do this to the site, which runs fine otherwise?
so what are some of the reasons i might get this type of "error"

Sorry, I have no idea. Clearly nothing has changed in ChronoForms unless a fie has been corrupted; so something else must be different.
The link looks as though a date & time has been added on the end. ChronoForms doesn't do that by itself.

PS I no longer remember if dashes are a problem with form names in J 1.0; they are in ChronoForms for J 1.5
