Checkbox names

I've two independent checkbox fields, but they're named {check0}. How can I change the name of a checkbox field? I don't see the way. Thanks.

I got another problem now. I've changed my server recently, and now when I send a form with a upload file it works fine, but it shows a message "Advertencia - Error al mover el archivo", in english "Warning - Failed to move file". How can I avoid this warning?

Sounds as though the folder doesn't exist, or the Joomla! User doesn't have write permissions for it.

I saw the error. The code in Field names/allowed Extensions/sizes(KB) of File Uploads tab was twice, then it showed a warning the second time that it tried upload the file. Thanks, you gave me the hint.
