Form showing error when submitting

When I fill out the form on this page http://www.makeoverfitness.com/home-gym-consultation-form.html I am receiving a red box at the top of the page with the number 1.

Even when I remove the file uploads from the form it still shows the error

Have you also set 'Enable uploads' to 'No' on the Form Uploads tab?

Ok I turned it off and it is working now. Now I got to figure out how to make the form upload work properly.
