client- contractor verification form

i need to create a form that clients fills out and submits from a (contractor/members) profile. Once that form is submitted the information is emailed to the owner of the profile (Contractor). once the profile owner verifies the information on the form, they click a predefined link in the form which says the information is legit and submit all the details to the administrator via backend or email. is this possible?

can you point me in the right direction.. maybe a keyword to look up? i have no problem using the search feature first, i just dont know what keywords to use in the search. All help is greatly appreciated
