Suggestion:
It would save a lot of confusion and grief if in Main Settings > Form Name instead of just
"Form Name: must be unique between forms" it said "Form Name: must be unique between forms - Do not include dashes."

From one of Bob's posts): Please rename your form form Contact-Us to Contact_Us or ContactUs or just contact. Using dashes in the form name will cause this problem.
