Check and submit FAQ

My problem is a bit complex and I hope I can help.
I must make a n°12 FAQ (the answers are not visible). Next to each Faq/Request must be a check button, the registered user can then select 3 max on click will submit applications with the answers in your inbox accompanied by a welcome message.
You think you can do?
Any ideas?
I do not know if I was clear and I apologize for my imperfect English.

Hi Enrico,

This is all straight-forward apart from the Maximum of Three boxes. There is no built-in way to check this in ChronoForms but you can do it in JavaScript without too much difficulty.
