form not recording check box selections

Not sure what i'm doing wrong. I created a simple form using the wizard. i connected it to a db table. when i fill in the form all my data saves in the table as expected EXCEPT my checkbox selections. I've searched the forums and found some posts that say to make sure the checkboxes are setup for arrays and AFAIK mine are. here is a link to my form:

The form HTML looks fine. Do you have "ChronoForms handle my posted arrays:" set to 'Yes' in the form General tab?

yes i do. what else can be wrong? all my other data (non check-box)is recorded as expected. i'd really like to get this working. thanks.
