Does anyone have a link to a guide or migration script for moving forms from a Joomla 1.0.x installation to 1.5.x? Did a quick search but nothing that really answers the question.

As far as I recall there is no migration tool. Adn there are quite big differences between the setup and features of the last Joomla 1.0 release and the current release.
The simplest way is probably to open both sites in different browser windows and copy and paste the settings between the two. It doesn't take long. Then you will need to tune the new forms to make sure that they work OK.

I have a substantial form created in the 1.0 version (2.3). The form is built completely by form code. In the 1.5 version (v4) I am able to place the formHTML code in the new version under the code, but am confused on how to add the OnSubmitCode - after sending email or where to add that. Can you provide some insight on how to do this. I was really hoping I wouldn't have to recreate this huge form.

If you use the Advanced Wizard in CFv4 you can click the 'Events' tab and drag various actions into the OnSubmit event to process your form data.
