I need someone to create some forms or at least the first one. Based on a radio button (and preferably using AJAX) a different set of subquestions appear. For example, if the user selects option a, one set appears, option b, a different set appears, etc. The form should prefill including the subquestions, if that is possible. Also, data must update or write the record to a DB based on whether that user has an existing record. I may require some JS calculation on the front but would be willing to discuss options. I do need some basic validation for numeric values. I like the Chrono platform but this one is a little complex for me and time is short. Please adv if you can tackle this project. I can build the forms. I know exactly what questions need to be asked. I know the output that must be returned. I even have a pretty good idea what needs to be stored in the db to make that all happen. However,I am really stuck on the logic of getting it in/out of the db. I am happy to give you more details by phone, email, pm, or whatever.

At this point I just need to know whether you are interested and have the time to create the first form of 25 questions. I can replicate the logic, field naming conventions, validation, etc. for all the other forms once I learn how you do it. Hope you can help. Doug
