Here is the working link from the Forms Manager:
http://modellabs.com/architectural-models/index.php?option=com_chronocontact&chronoformname=Free_Quote

However I cannot get a menu item to link to it using the Menu Item Type > Chronocontact

If you try and click on the menu item Free Quote, the page states, "There is no form with this name or may be the form is unpublished, Please check the form and the url and the form management"

There is a green check under the Publish column.

My other form, contact, and respective menu link work fine:

The footer menu link "Contact Us" works.

Please help solve this.

I resolved this by deleting all menu items that linked to the free_quote form and recreating them.

Usually this is because there is no form name in the menu config box.
