Hi,
I want to use in my form about 10 RadioButtons on the last item, the user should write their own words/text.
I hope you know what I mean.
How can I do this?

The last item "other" should be a text filed but with the ratio-button in front.

Is there a possibility but two lists of radiobuttons side by side?

Thank you very much, I have done it.
But I have another short question. I use the ja_purity template and your modul. I created a html form and everything works great.
But I only want that the title of the page is shown on the top, like I have on the other sides, when I clicked at it in the mainmenu. I tried it with <title> in html, but it doesn't work.

I have already activated "show the title" under the systemparameters of the mainmenu.

Can you tell me, what I am doing wrong?

thank you very much!
I put your code in front of my html-code. See my first screenshot. "It is in the bottom of the side"
AND
under the point mainmenu the title of the page is activeted as before. See my second screenshot.

I also give you another screenshot, so I hope you can understand what I mean with showing the title on the template.

Sorry I missed the post . . . but I just spent several minutes looking at it nad I have no idea what the problem is. Something about the page title. But you haven't added any code to change or display the page title that I can see.
