Upgrade blew up the form

We are using Joomla 1.5.9, sh404sef, and had been using ChronoForm 3.1 RC2.
We upgraded to RC5.5 and when we tried to save the file we got.

TableChronoContact::store failed - Unknown column 'stylecode' in 'field list' SQL=UPDATE `jos_chrono_contact` SET `name`='annualreport',`html`='

Do I need to update a table?

Well, the "Upgrade SQL and load Demo" solved the first problem.

Now our form is only missing the "Yes/No" radio buttons.
Any ideas on how to get those back without rebuilding the form?

No - I can't think why they would have disappeared, ChronoForms pretty much treats the Form HTML as a text string and doesn't normally chop bits out of it.

What is in the Form HTML where they should be?
