Tho Whom it may concern:
I have imported an old form table into Chrono Forms and have successfully recreated the look and functions of the original form in the new web site that my company is creating for our customer. The problems I face are the is no user inputted data is being registered in the database table I created for the form. Also, no emails are being sent from Chrono forms after the form is submitted. I did not create the original form or the scripts it uses in conjunction with the web service provider. The new form does work as it did on the old web site, but my customer desires the inputted data saved in a database table and an email sent out to the user when they submit the completed registration form. I have attached a backup of the form in a zip document for your review. Thank you kindly for your time.

Scott George
