There is no form with this name or may be the form is unpublished, Please check the form and the url and the form management

I get this error when attempting to invoke the form from a standard Joomla (Main) menu.

If I invoke the form from Forms Management, it works; however, I noticed that it displays on the home page template - not the secondary page template.

My sense is that may be the key to the problem, but I have not found a way to prove it.

My form name is "Prayer_Request", and it is the only form (no duplicates); therefore, the common causes are not the issue this time.

Any assistance will be greatly appreciated.

Are you using any kind of SEF and/or .htaccess mod_rewrite rules?

I'm having the same problem...

we are using sh404SEF WITH the .htaccess file

however I still get the error when sh404SEF is turned OFF so that pretty much means it has to be in the .htaccess file huh...

any ideas what I'm looking for...??

whats your form name ?

can you access it fine from the link in the forms management ?

did you enter the form name in the right side box when creating the menu item ?

I'm getting this message too, with my very first form. I've followed the tutorial, set Debug to Yes, and clicked on the link in Forms Management. The webpage comes up, no form, but this message. I can't see any step I've missed from the tutorial. What have I missed?

Did you remember to publish the form in the Forms Management?

Don't jump your horses just yet. Incorrect mod_rewrite rules (.htaccess) can cause several odd issues. However, that does not rule out other sources, it's just good to know if these are in play. Disabling SEF alltogether however, suggests that the error lies in other places.
First off, could you verify Max' suggestion?

Yes, it was published in the Forms Management.
I've since found that it works if I don't use dynamic fields. That's ok for me.
