odd results for radio group

I've got a radio button group with five options asking how people found out about the site. It's not a mandatory field.

I've looked at the results for the last week and there are 'yes' and 'no' responses in the table and these are not an option I have listed

I've searched the forum and nothing so think I've got an issue related to my database...

ideas thoughts?

ok looks as through I'm getting the results from another page in this field.

Looks as though this is happening because I have a radio group first on each page. How do I fix this?

Please contact me through the "contact us" page and provide login to your website to check the form, I think its something wrong some where with your form, hard to tell here!
