Simple Form+2 Date Pickers?

Hi,

I have a simple form question...

What is the easiest way to create a form in Joomla 1 to take a persons name, address, phone, etc and two dates (hopefully by using a calendar) a start date and a leaving date?

Hi,

You can create this form with chronoforms very easily, make this form in any html editor or just right click this webpage you got the image from and choose "view source" then copy and paste the html code and any JS code if you like then use it to create your form in chronoforms in 5 minutes, of course 5 minutes if you really know how the things will go, for the calenders, you can do this easily if you refernced the joomla calender in the php part of the code.
