Error message "You have entered a wrong security code, please retry!"

Not sure what version of CF - but i haven't updated in the last month or 2.
When i attempt to enter the captcha code I always get this erroe message:
"You have entered a wrong security code, please retry!"

It was working before - there are comments on the site.
Suddenly stopped...any suggestions welcome.

Think I can see what is wrong - the captcha code is not very clear.
It has the letters "xt3WN" in black over the code in light grey - one therefore can not see the light grey code behind the black letters.

Any suggestions how to fix this?

As far as I know all the Captcha images are the same unless the chrono_verification.php file has been hacked - can you post an screenshot of what you are seeing please.
