Can't find error

I'm just struggling with a form, and to be honest, I don't believe it's the system... but me and I could really use a second set of eyes. I had a request to edit a "working" form - and now I cannot seem to get it to work properly and I've gone over the code sooooo many times, my eyes are bleeding.

I had multiple changes to the form, so I tried making them one group at a time in order to try to locate the error. It seems to be happening on a group of "selects" I've added to the form. What is happening is two things... when sending the form instead of going to the Thank you page, it white screens (sure indication of a code error). Secondly it does send a perfect email, but does not collect the data into the db as I have asked the system to do.

I've emailed two files for you to take a look at - the form.bak - and a file with my login info and the db table to take a look at.

(It's a complicated form in regards to the amount of info that is being collected, but I think I can't see the forest for the trees anymore.

Any help would be greatly appreciated.

I'll take a look but it may not be this evening (10pm here).

If you get a white screen set the site error reporting to max and you should see a PHP error that may be some help (rest it quickly if the site is live).
