Need multi-page form created

I need to have a 5 Step process created for a website template design. The step will be:

Step 1- choose a template that will have radio group buttons with the template thumbnail image next to it. (12 templates)

Step 2- register form that will store to joomla register DB table.

Step 3- need to have 5 fields which are: website name, website url, website description, website keywords and image uplaod for website logo.

Step 4- Need to have fields for Page title, page description and page image upload. I will need 10 section of the same information, for example:
page1 title page1 description page1 image
page2 title page2 description page2 image
page3 title page3 description page3 image
etc....

Step 5- Price for package and checkout button that will go to paypal for process.
NOTE: not sure if its possible but could step 4 be able to select a 5 page package or 10 page package that will show a different price on step 5 checkout page.
Need to have this done ASAP, what will it cost and the time frame to complete it.
