it worked a few times but now when it is submitted I get "You are not allowed to access this URL" someone please help.

Remove it if there is one. After you submit the form shows a 'not published' error message with an odd url. I don't know what's causing it just that it doesn't look 'normal'.

PS I suggest that you debug this with a very simple test form. In general I don't have time to test long forms with validation - it just tkes too long to fill in the fields correctly.
