Having problems with the forms. When I click FORMS MANAGEMENT, I get INDEX.PHP. It's the only menu where it occurs. I've uninstalled it and re-installed it but still same problem.

An version of the forms is installed on a different server with one of my clients, but dont have that problem.

Presulmably this is with ChronoForms not ChronoConnectivity (this is a ChronoConnectivity forum).

Is this a new installation or an upgrade? If it's an upgrade please clear your browser cache so that any old code is cleared.
