Form is messed up

Hi guys, I have just stared using ChronoForms

Ok here is the problem, if you look at this page

you can see the fields and lables are not in the right order and messed up, I worked around CSS and no success,

Do u have any idea why is it like this?

It looks OK here (in FireFox) except that you have half-a-dozen empty paragraphs before the form.
