I made another form but with the same results:

Perhaps there are server requirements? I'm with hostgator and it's worked fine with other components.
Is there a debug mode that would help?

Hi Rowby,
There is some intereference or conflict with other code being loaded in the template. If you view the form without the template everything is OK. There's no JavaScript error reported in FireBug so I'm not sure exactly what the problem is, you could try turning other page elements off to see if you can isolate it.
