I have:

ChronoForms Plugin 3.1RC5

I upgraded ChronoForms from (I think) 3.1RC1, and ran the 'Upgrade SQL' and also did these mods.
The original form didn't display, so I created a new one, which didn't display either. The form is attached.
Help me Obe-Wan Kenobe, you're my only hope.
Edit: I am trying to access the form from: here. And in my page I have the tags {chronocontact}contactUsForm{/chronocontact}.

Hi speludner,
Well, the form is OK - you can see it here so either the ChronoForms Mambot/Plugin isn't published or it isn't the current release and doesn't work with the new version of ChronoForms.

Hi Bob,
Thank you so much! Just knowing the form displayed OK allowed me to focus on the Plugin.
In the end I uninstalled (and deleted its entry in the database) and re-installed it and it works perfectly.
Thanks again. I also want to say how much I appreciate getting a response on a Sunday!
